General Purpose
Provide physician level care for HealthSpring membership enrolled in the complex care program.
Core Responsibilities/Essential! Functions
The complex care physician will participat! e as a part of a multi-disciplinary team providing care to medicare patients in a program designed to fundamentally change the manner in which health care is delivered. The complex care physician will provide physician services including advising and counseling patients regarding the most appropriate setting to meet their healthcare needs, performing physician assessment and evaluation of their problems, and providing the appropriate information regarding specialist services, and end of life programs. The clinical role includes participation in the development of appropriate medical management strategies with a goal to maximize medical therapy and ensure proper pharmaceutical and home health services to maintain health status.

Work Schedule
Monday - Friday; 8 AM - 5 PM
Complexity and Scope
Detailed knowledge of chronic disease management programs and services. Supervisory experience, organizational skills and excellent communication abilities are necessary. Marketing and public relations skills are required. Knowledge of insurance/managed care processes is required. A high level of diplomacy and creative problem-solving, priority-setting, and critical-thinking skills are necessary for operations in the outpatient clinic. Experience with the management of Medicare and Medicaid populations. Ability to work closely with nurses, behavioral health specialists, pharmacists, case managers and social workers- all onsite and part of the care team,
Physical Demands/Environmental Factors/Core Competencies
Ability to work in a medical office setting.
Focuses on the management and improvement of he! alth for the frail seniors and members with chronic disease in the popu! lation.
Documents all diagnoses with the highest degree of specificity, ensuring that all aspects of disease progression and manifestation are documented annually.
Attends to the frail elderly and chronically ill experiencing episodes of acute exacerbation and monitors disease progression.
Accepts same day/next day or urgent appointments for patients discharged from the acute setting in need of intensified physician or case management services to ensure acute episode has resolved and the patient is not readmitted.
Supplements Primary Care Provider Disease Management activities to stabilize patients and gain control of disease processes as a means of avoiding unnecessary hospitalizations.
Assists with the introduction and discussion of disease progression and the need for making determinations with regard to end of life choices so that patients’ wishes can be documented and addressed.
Provides orientation and education ! relative to Hospice enrollment as a care alternative for the patients that meet criteria for hospice services.
Participates in multi-disciplinary meetings and recommends treatment alternatives designed to keep patients healthy or resolve on-going clinical issues.
Communicates all services provided in the BHACC with the patient’s Primary Care Physician. Discharge plan for continuing care management is developed and communicated to ensure continuity of care.
Assists with the management of skilled nursing facility and hospice patients as requested.
Perform other duties as required consistent with complex care and disease management services provided to patients.
Minimum Qualifications
Active and unrestricted medical license in MD
Board certification in internal medicine or family medicine with geriatric fellowship
ACLS certification
Long-term interest in Bravo Health Advanced Care Center
