Responsibilities:
Assists the Director of the Pharmacotherapy Management Center in assuring efficient and effective operations of the Nutrition Department.
Performing telephonic therapeutic consultations with patients and/or prescribers.
Performing data collection of member’s lab values, diet, and medical history.
Completing a full nutrition assessment for patients.
Develop a plan of care detailing actions for the patient and/or prescriber based on the nutrition assessment.
Collaborate with the interdisciplinary care team to develop an appropriate plan of care for patients.
Documentation of the plan of care into the data base.
Participation in continuing education sessions provided by the department.
Effectively participates in and contributes to staff meetings and interdepartmental work groups.
Responsible for facilitating all department first-level complaints and/or issues and resolve the! m when possible, prior to forwarding them to the Pharmacothera! py Management Center Manager for resolution.
Identifies areas where systems modifications could result in operational improvements and report those to the Director of the Pharmacotherapy Management Center.
Develop patient education materials related to the most common disease states encountered.
Develop and deliver educational sessions for Pharmacists and Nurses where appropriate.
Participates and collaborates with the Director of the Pharmacotherapy Management Center on CMS regulatory reporting and other projects as needed.
Participation in Quality Assurance projects.
Maintain a high level of clinical knowledge of chronic disease states and their management.
Strong communication skills essential.
Other duties as assigned

Requirements

Qualifications:
Registered Dietitian, with licensure in Maryland within 6 months of starting. Additional state licensure may be required.
Strong clinical skills are essential. Three years of experience in a Clinical Dietitian role in an acute care or long-term care institution.
Proficiency in using database information systems and Microsoft Office Suite.
Ability to research and analyze the medical literature including drug information, disease states and clinical practice guidelines.
Excellent verbal communication skills.
Excellent team work skills.

Preferred Qualifications:
Masters in Science in Nutrition/Dietetics OR 10 years of equivalent clinical experience.
Board certification preferred (CDE, CNSC, ect).
Experience with the geriatric population
