SUMMARY: This is a specialized professional licensed Pharmacy Departmen! t position which applies the principles of pharmacokinetics and pharmacotherapeutics in drug therapy monitoring and medication consultation and serves as an educational resource for the medical staff, pharmacy staff and other hospital and community groups to ensure appropriate, safe and efficacious medication utilization.
The associate must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status. The pharmacist must demonstrate knowledge and skills necessary to interpret appropriate information needed to identify each patient's requirements relative to his/her age-specific needs and provides care needed as described in the department's policies/procedures.
Required Education:
- Graduation from an accredited school of pharmacy with a six (6) - seven (7) year college program awarding the traditional PharmD. degree.
- Licen! sure as registered pharmacist by the Maryland State Board of Pharmacy. (Required)
- Achieves and maintains required skill recognition status within six (6) â€" twelve (12) months of initial hiring.
- One (1) year hospital pharmacy experience preferred.
- Knowledge of ASHP Guidelines for Institutional Pharmacy Services.
- Management of Pharmacokinetic Service in Acute Care Institution and/or other designated pharmacy sponsored service.
- Assists in the instruction of Pharmacy students completing clinical pharmacy rotations at SAH.
- Coordinates and supervises an established undergraduate clinical rotation program.
- Coordinates and supervises a postgraduate residency program.
