Role Profile

The Clinical Pharmacist assists with the development and execution of numerous initiatives that improve the quality and cost effectiveness of drug therap! y for HealthSpring members. Initiatives include coordination o! f medication distribution with community pharmacies, development and administration of medication therapy management services, education programs for staff and patients, as well as completion of managed care pharmacy processes, such as prior authorizations. The goal is to enhance relationships with physicians, staff, and HealthSpring members and to address issues in the clinic environment, including medication compliance, disease management, specialty drug utilization, patient education and member satisfaction.

Essential Duties and Responsibilities

Conduct population analysis of member medication management, including but not limited to medication adherence and compliance.

Develop medication therapy management programs for identified members related to anticoagulation, hypertension, hypercholesterolemia, diabetes, etc.

Coordinate with Management Team to assist with the medical management of their patients as necessary.

Schedule a! nd conduct appointments with members as needed.

Assess therapeutic needs as well as evaluate and monitor therapy regimens in conjunction with the health care team to ensure safe and effective use of medications.

Provide member medication evaluation and recommendations to physicians through the Electronic Health Record.

Organize and document medical interventions and provide reports tracking the progress and outcome of the interventions.

Review and complete requests from HealthSpring members and clinic physicians for therapies requiring prior authorization.

Provide consultation to clinic staff including but not limited to formulary, pharmacy benefit, safety limits and step therapy edits.

Recommend medication alternatives to clinic staff as appropriate.

Develop and present educational programs for clinic staff regarding disease states, medications or medication class reviews, and clinical guidelines.

Hand! le phone inquiries from physicians, the clinical staff, and HealthSprin! g members.

Develop and distribute patient education materials.

Participate in external community outreach program activities.

Other duties as assigned by management.

A year or more experience in a clinical setting.

Licensed Maryland Pharmacist with an interest in population health management and managed care.

Managed care pharmacy experience preferred.
