Clinical Coordinator (CC)

Location: Baltimore, Maryland

Description: Pharmacare, LLC is at present recruited Clinical Coordinator (CC) right now, this position will be designated in Maryland. For complete informations about this position opportunity please read the description below. The Clinical Coordinator serves to coordinate prescription related services between Pharmacare and the various clinics and hospitals that Pharmacare partners with. This position reports to the Chief Operating Officer and involves complete responsibility and coordination of effort among various team members including:

The Pharmacy The patients The provider and the clinics Delivery personnel Couriers UPS Pharmacare Clinic Technicians posted at these clinics, and Support staff.

The primary responsibilities include (but are not limited to) the following:

1) Prescriptions :
i. New prescriptions : Establish clinic specific protocols to ensure that all new prescription orders from these clinics are received by specific daily deadlines at designated locations (manual or E-fax)

ii. Prescription Refills :
iii. Establish proper clinic specific protocols for handling refill requests from patients iv. Establish proper clinic specific protocols for requesting prescription refills from providers v. Track refill status for each clinic patient and work proactively with providers to ensure patients have necessary refills available when requested.

2) Prescription filling : Collect, print and forward all new prescriptions and requests for refills to the pharmacy for processing and filling. Ensure that all prescriptions are reconciled and accounted for by end of each day.

3) Prescription deliveries : Coordinate delivery protocols to ensure on time delivery of prescriptions. This includes the following:

i. Delivery tracking log: Ensure that the delivery tracking log is completed and maintained up to date at all time. ii. Patient demographics: Maintain an accurate demographic database for all delivery patients. iii. Ensure that all filled prescriptions are marked for delivery as appropriate (clinic Vs home delivery). iv. Ensure that these prescriptions are picked up at scheduled intervals from the pharmacy. v. Ensure that each prescription is documented on the delivery log. vi. Ensure that all deliveries are distributed to designated couriers for on time deliveries. vii. Ensure that all deliveries are completed and reconciled by end of each day, and in compliance with established delivery protocol. viii. Ensure that medications are transported in a safe and secure manner by delivery personnel ix. Ensure that all medications are stored per manufacturer recommendations during transport. x. Ensure that each delivery is accounted for by end of each day. xi. Establish and monitor protocols for collection of prescription copayments and waivers. xii. Ensure that pharmacist counseling is offered to all delivery patients xiii. Ensure that DOT (Department of Transportation) compliance documents and mileage logs are placed in each delivery vehicle and completed per Pharmacare policy. xiv. Establish appropriate measures, checks, balances, and protocols to ensure that all deliveries are made as promised and on time. xv. Collection and reconciliation of prescription originals from providers and patients. xvi. Coordinate and resolve any emergent delivery issues as appropriate.

4) Returns, picks ups, etc:
i. Ensure that all undelivered medications are returned to the pharmacy in a safe, and secure manner, and with appropriate storage compliance. ii. Ensure that all returned medications are reconciled on the delivery log and forwarded to the pharmacy for proper processing and credit iii. Arrange for pick up of any original prescriptions from clinics, providers, and patients, and reconcile them with fax / telephone orders. iv. Arrange for pick up of any returns of prescription medications (arranged by prior approval of Pharmacy Manager) from Clinics, or patients

5) IPAC (Injectable Psychotropic Adherence Clinic): Coordinate IPAC protocol to track patient discharge, scheduling, prescription filling, administration, and follow up.

6) HIPPA Compliance: Ensure that all prescription deliveries are made in compliance with HIPPA regulations

7) Communication liaison : The clinical coordinator will

i. Serve as a liaison with corresponding Clinic Coordinators and providers in various clinics and hospitals in regard to prescription related issues such as clarifications, drug interactions, drug substitutions, prior authorizations, follow up, etc ii. Serve as a liaison with patients regarding their deliveries, prescription refills, complaints, etc. iii. Serve as the primary liaison with Pharmacare representatives at various clinics such as Mosaic, BBH, etc to coordinate their activities and functions iv. Serve as a liaison with delivery personnel, couriers, and UPS

8) Record keeping :
i. Document and track all new prescriptions received from each provider via an ongoing tracking log. ii. Document and track refill status of patients to ensure availability of refills iii. Document and track prescription delivery status.

9) Reporting :
Provide daily clinic specific report to corresponding clinic coordinators and the Pharmacare management team. These reports would include the following information: 1. Daily census 2. Prescription fill status for each patient 3. Refill status 4. Delivery status 5. Any outstanding issues

10) Customer service and satisfaction : The CC shall develop measures to monitor and assess quality of customer service to clinics to ensure a high level of customer service and satisfaction.

11) Clinic Technician (CT) : The Clinical Coordinator will be responsible for managing and coordinating the activities of Pharmacare Clinic Technicians posted at various clinics and hospitals. This would include (but not limited to): i. Coordination of communication between the clinic and the pharmacy ii. Transfer of new prescription orders and refill requests to the pharmacy iii. Distribution of completed prescription orders to clinic patients iv. Resolution of complaints

12) Problem resolution : Ensure that any problems and issues pertaining to clinic prescriptions, refills, deliveries, complaints, etc or tracked, followed up, and resolved promptly.

13) Meetings :
i. Schedule and coordinate meetings with corresponding Clinical Coordinators and providers as appropriate. ii. Schedule and coordinate meetings with support staff (see # 13 below) as appropriate. iii. Provide weekly updates at the weekly Pharmacare Leadership meetings.

14) Supervision & Management :
i. The Clinical Coordinator will have direct responsibility for the following personnel positions and as such will develop, monitor, evaluate and coordinate their day to day activities and job function:

1. Assistant Clinical Coordinator 2. Delivery coordinator 3. Delivery personnel 4. Support staff 5. Pharmacare clinic technicians

Ii The Clinical Coordinator will perform annual and semi-annual performance evaluations of the support staff referenced above. Iii The Clinical Coordinator will develop and enforce performance assessment tools and measurements including audits, inspections, etc to assess performance of the support staff, and take appropriate corrective measures when needed to ensure optimal job performance and compliance with Pharmacare mission and vision.

15) Duties as assigned: In additions to specific duties identified above, the clinical coordinator will assume and participate in any relevant tasks and responsibilities as deemed appropriate by Pharmacare management team and in keeping with the overall mission and scope of Pharmacare Discount Pharmacy.

16) Staffing Hours: This is a full time staffing position with on-site hours appropriate to ensure proper supervision and outcome of all aspects of this position.

Requirements- Applicants must be at least 21 years of age. Applicants must be able to read, write and speak the English language. Applicants must be physically and mentally able to safely perform the essential job functions without obvious risk of injury to the employee or co-workers. Applicants must receive satisfactory results from a driving background check, criminal background check as conducted in accordance with applicable laws; and must pass a drug screening. Applicants are required to work a full time schedule 5 days a week not to exceed 40 hours and work one weekend per month.
