ABOUT THE JOB
Evolent Health is seeking RN care managers to be founding members of the Population Health team for our local client MedStar Health System. Our goal is partnering with the nationâs leading health systems in their strategic evolution to positively impact and improve the healthcare delivery system. As a member of a team of nurses, social workers, registered dieticians, physicians, pharmacists, and program coordinators, you will have the opportunity to make a profound impact on the lives of people living with multiple chronic illnesses. You will connect with your patients in person, on the phone and through email, in the hospital, and in the physicianâs office - essentially however and wherever the patient needs your assistance to improve their health, better understand their illness and coordinate their care.
What You Will Be Doing:
- Along with other members of the Population Health team, conduct comprehensive assessments that include the medical, behavioral, pharmaceutical and social needs of the patient, identify gaps in care and barriers to attaining improved health.
- Based on this assessment, and in conjunction with the patient, the patientâs physician and other members of the population health team, create and implement a care plan that will address the identified needs, remove the barriers and improve the health of the patient.
- Coordinate care by serving as the contact point, advocate and resource for the patient, their family and their physician, building effective relationships through trust, respect and communication.
- Assesses the patientâs knowledge of their clinical condition and provide education and self-management support based on the patientâs unique learning style.
- Measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.
- Bachelorâs Degree in Nursing
- 3-5 years of nursing experience, preferably in home health, case management or physician office, coordinating care across multiple settings and with multiple providers.
- A current DC, MD or VA Registered Nurse License.
- CCMC (Commission for Case Manager Certification) â" a plus
- Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion; fluency in a foreign language is a plus.
- Strong work ethic built on a foundation of proactivity and teamwork.
- Ability to navigate ambiguity with the aid of structured problem solving techniques.
- Committed to the practice of inquiry and listening.
- Home-based position with a flexible work schedule.
- Computer proficiency in Microsoft Office.
