Location: Baltimore, Maryland

Description: University of Maryland Medical Center is currently seeking to employ Clinical Pharmacy Specialist - Transplant right now, this job will be assigned in Maryland. The Pharmacy Clinical Specialist provides collaborative pharmaceutical care to heart and lung transplant patients but who will also work wi! th the abdominal transplant pharmacy team.

Primary responsibilities of this position include:

  • Participation on a multi-disciplinary team daily rounds
  • Working with the physician to individualize specific drug regimens via a collaborative practice agreement.
  • Providing education for transplantation related studies to nurses and pharmacy staff
  • Implementing and monitoring clinical protocols and guidelines.
Other duties include:
Promotion of rational, cost-effective drug therapy, education of pharmacy students and residents, representation on medical center committees, and collaboration in research initiatives.

The ability to instruct and train pharmacy residents and students and acts as a mentor to clinical pharmacists. Must plan, organize, complete and evaluate assignments and projects and provide oversight to others where delegation is required. Demonstrated ability to affect pharmaceutical ! care outcomes.

Bachelor of Pharmacy from a college or! university accredited by the American Association of Colleges of Pharmacy is required. Master of Science, PhD or Doctor of Pharmacy from a college or university accredited by the American Association of Colleges of Pharmacy is preferred. Five years of clinical pharmacy experience required with Bachelor's degree.

Current licensure by the Maryland State Board of Pharmacy is required

Completion of an ASHP accredited pharmacy practice residency, specialty residency or a fellowship; or five years relative clinical experience required. Completion of a PGY2 Residency is preferred.

Certification from the Board of Pharmaceutical Specialties is preferred. If specific practice involves the Anticoagulation Clinic, then certification as an Anticoagulation Provider is preferred.
