The Hospitalist physician provides clinical care to health plan members. The Hospitalist physician participates as a part of a multi-disciplinary team providing care to mem! bers. Focuses on the management and improvement of health for ! members with chronic disease(s) in inpatient and outpatient hospital settings. Provides physician services, including clinical care, advising and counseling patients regarding the most appropriate setting to meet their healthcare needs, performing physician assessment and evaluation of problems, and providing the appropriate information regarding specialist services and end of life programs. Participates in the development of appropriate medical management strategies to maximize medical therapy and ensure proper pharmaceutical and home health services to maintain health status.

Clinic Setting : Diagnoses and treats frail elderly and chronically ill patients experiencing episodes of acute exacerbations and monitors chronic disease progression in an urgent care setting with cardiac monitoring and IV therapy as appropriate. Supplements Primary Care Provider Disease Management activities to stabilize patients and gain control of disease processes as a means of a! voiding unnecessary hospitalizations. Documents all diagnoses with the highest degree of specificity, ensuring that all aspects of disease progression and manifestation are documented annually. Participates in multi-disciplinary meetings and recommends treatment alternatives designed to keep patients healthy or resolve on-going clinical issues. Communicates all services provided with the patient's Primary Care Physician. Discharge plan for continuing care management is developed and communicated to ensure continuity of care. Actively engages in various clinic initiatives that are supported by the Health Plan. May assist with the management of skilled nursing facility and hospice patients that seek care in the medical clinics. Works closely with onsite Behavioral health, social work, and pharmacist staff to provide comprehensive care.

Inpatient Hospital Setting : Serves in a hospitalist function. Responds to member presentation after Emergency Department work! -up to determine the appropriate level of care, including admission, ob! servation, or facilitating transfers to outpatient care, or alternative providers (SNF, Home Health). Provides inpatient care as the attending of record, takes “call” as required, manages specialty engagement, meets with the family to address care issues. May provide utilization review onsite of inpatients. Works with facility care managers to provide appropriate and timely use of resources. Participates in utilization review meetings. Responsible for transitional care coordination as needed for discharged patients, ensuring continuity of care at the next level of care. Appropriately refers patients to other teams within the organization such as care management and others as needed. Appropriately documents relevant information regarding inpatient stay of patients in the EMR. Rotates care between inpatient and outpatient facility. May act as Team Lead.

Qualifications : Board Certified Internal Medicine Physician with an active and unrestricted medical lic! ense and an unrestricted DEA license (in applicable state of practice setting). For Inpatient Hospital care, admitting privileges at appropriate hospital. ACLS certification. At least five years experience in a health care related setting. Prior geriatric experience. Knowledge of chronic disease management programs and services and insurance/managed care processes and populations. Adept at marketing and public relations. Ability to travel within market locations.

Preferred Requirements : Background in inpatient care, geriatric, and/or palliative care. Experience with electronic medical record system, physician’s practice operations, health plan operations, or medical management setting.

Schedule : Rotating schedule between Hospital (80%) and Advanced Care Center (20%)
