Union Hospital of Cecil County is looking for a Clinical Pharmacist who will be accountable for providing balanced and comprehensive medication and medical information services in response to inquiries from physicians, nurses and other healthcare professionals. The essential job functions will include the following:
- Responsible for interpreting and answering medication and clinical related queries through the interpretation of published and unpublished medical literature in conjunction with the application of clinical pharmacy skills and knowledge
- Informati! on must be provided in a scientifically sound manner while rec! ognizing and being responsive to issues that may impact UHCC legally
- Ability to work independently yet cooperatively with Director of Pharmacy, clinical coordinator, pharmacy staff, and hospital staff
- Provide in-services for Nursing, Medical Staff, and Pharmacy
- Monitors medication therapy regimen for contraindications, medication-medication, medication-disease, and medication-food interactions, appropriateness of medication and dose
- Performs duties of staff pharmacist as required
- Monitors Creatinine Clearance for renally cleared medications
- Collects and interprets Adverse Medication Reaction data and prepares the summary for quarterly Pharmacy & Therapeutics Committee meetings
- Documents all clinical activities and interventions accurately and completely
- Maintains strict confidentiality of patient information
- Available to provide pharmacist coverage when needed.
- Orders and monitors INR/! PT/PTT levels for anticoagulants
Licenses: Currently licensed to practice Pharmacy in Maryland.
Experience: Hospital experience, extra qualifications (BCPS, CGP, etc.), residency and fellowship preferred, but not required.
Work Schedule: Minimum of 40 hours per week, but hours may be long and irregular; includes evenings, weekends and holidays as necessary
