The Pharmacist-WOW assures the appropriate selection, dosing, administering, and dispensing of medication orders.
Responsibilities:
Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.
Recommends formulary alternatives when appropriate.
Compounds and dispenses medications for accurate ! delivery to patients.
Provides drug information and educat! ion to medical staff and patients.
Communicates with physicians and other clinicians as necessary to facilitate care.
Education & Experience:
One (1) year hospital pharmacy experience preferred .
Graduate of an accredited school of pharmacy with a five year college program in pharmacy or a six year PharmD program at date of hire required.
Licenses & Certifications:
Current licensure as a registered pharmacist by the Maryland State Board of Pharmacy at date of hire required.
