Confident, competent practitioner able to recommend and follow through on a medication regimen for inpatients based on principles of successful glycemic management
Assesses knowledge needs in patient/family in diabetes self-care and delivers resources/education to fill those needs.
Uses assessment tools to quantify nursing and physician education needs in glycemic control, nutrition needs of the inpatient diabetic
Conducts education sessions for new and seasoned nurses to enhance clinical judgment skills
Recommends clini! cal practices for diabetic inpatients in the daily practice of! nursing and support
staff
Initiates structured education sessions for physicians, including guest speakers, to enhance glycemic management skills, new therapies in the management of diabetes, prevention and treatment of disease complications
Participates with department colleagues in monthly outpatient DSME program

acilitates team resources, including Physician, RN, Pharmacist, Case Manager, Podiatrist
(if needed) in making rounds on complex diabetic patients
Proactive in communicating successes /opportunities for improvement to nursing staff
Qualifications
Must have an active RN license
Certified Diabetes Educator
required
A minimum of 5-8 years of related experience required
Must be competent with computer applications including EMR
Candidate must be well -organized
Must possess the ability to collaborate effectively with physician and ! nursing staff
Confident in ability to teach self-management skills to diabetic patients
Be committed to team efforts
