Pharmacist, Night Shift

MedStar Southern Maryland Hospital Center is actively recruiting for a fulltime Pharmacist to join our dedicated team! This position provides support for 10 hour shifts, 7 on 7 off. Night hours are 9:00pm â€" 7:00am.

Primary Duties and Responsibilities:

Review patient medication history and orders for appropriateness taking into consideration medical history, diagnosis, demographics, laboratory values, current medications, allergies, and other pertinent information.

Contact physicians regarding non-formulary medication requests, unclear orders, duplication of therapy or any recommendations to changes in therapy.

Perform order entry.

Perform therapeutic substitution of medications.

Organize daily operational duties in the dispensing function. Gives direction to supportive personnel.

Fill medication orders and sign off on work completed by pharmacy technicians.

Dispense narcotics to pharmacy technicians and other licensed hospital personnel.

Maintain inventory and records on narcotic distribution.

Resolve problems regarding orders for patients or anything that interferes with patient care or the normal flow of work in the pharmacy.

Perform medical record review for clinical rounds. Compare pharmacy profile to nursing medication administration record.

Perform IV to oral therapy conversions.

Recommend renal dosing adjustments of medications

Provide drug information service.

Resolve issues related to the pharmacy information system and dispensing technologies (Pyxis).

Participate in anticoagulation management.

Participate in performance improvement initiatives and collects data as required.

Compound sterile products according to USP 797 standards. This includes, but is not limited to, adult TPN, neonate TPN, PCA cassettes and chemotherapy.

Complete nursing unit inspections.

Perform other duties as related to the position.



Qualifications:

Bachelors Degree in Pharmacy; Doctorate preferred

MD State License

PGY1 Residency desired

1 year of experience required, preferably in a hospital environment.

Supervisory experience required
