We offer traditional home infusion, specialty infusion and respiratory services for patients with acute and chronic conditions. Each year our experience nurses, pharmacists and respiratory therapists provide compassionate, comprehensive care to approximately 120,000 patients in non-hospital settings.Job Duties Include: Seeks out and authorizes subcontracted nursing providers to provide home health / infusion care and services. Appropriately tracks and reviews clinical documentation from subcontracted providers. Provides clinical updates to appropriate Plan and Provider personnel in a timely manner as appropriate. Documents updates and communications within the clinical data base. Acts as a ready resource to staff to address clinical questions, process and/or procedure questions as they arise. Assists with orientation of new staff when requested. Acts as a Team Leader to a designated operational team to insure effective and efficient processing of referrals/orders. Assumes supervisory responsibilities in the absence of the General Manager as requested. Rotates with the other Care Managers to provide 24/7 clinical support to the after hours program's primary on-call personnel. Graduate of an accredited school of nursing with valid license in state of practice.Minimum of two years of experience as a health care professional, with at least one year in home care.Preferred Qualifications:Management and Performance Improvement Experience. Previous Home Care experience. BSN or advanced professional degree. Case Management Certification.
