This position will support current staff in providing clinical pharmacy services at both the Cambridge and Pokomoke Community Based Outpatient Clinics (CBOC's) and thus provide more comprehensive care to patients. Planned responsibilities include expanding chronic disease state management and anticoagulation clinic services where currently there is an unmet need for additional appointments. The incumbent would help expand clinical video telehealth(CVT)/telehealth services for veterans who live in remote areas and face to face or telephone visits for medication therapy management or anticoagulation services.The incumbent will support the Primary Care Clinic providers in achieving goals for chronic disease state management, promote adherence to the VA National Formulary and direct providers in the appropriate utilization of non-formulary medications only wh! en formulary alternatives are not appropriate. The CP will ensure that ! observed and historical adverse drug reactions are reported in the VA Adverse Drug Reporting System and documented in the patient medical record without delay which promotes patient safety. Patients prescribed medications would have the opportunity to receive face to face medication counseling by the CP for prescriptions entered by local providers. The CP will support the Medical Center Outpatient Pharmacy through the onsite electronic processing of prescriptions which would decrease patient wait times for mailed prescriptions and reducing parent medical center pharmacy processing. Additionally, the pharmacist will assure that medication storage and maintenance of the medication rooms located at both facilities comply with Joint Commission Standards. All other duties as assigned. QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
a. Citizenship. Citizen of the United States.
b . License. Full, current and unrestricted license to practice pharmacy in a! State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
c . Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
d. Physical Requirements . Scope of the Examination: The examination will be concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and will include requisite laboratory and other screening as required by Federal regulatory agencies
e. English Language Proficiency. Must be proficient in spoken and written English
GS-11 Pharmacist
(a) Experience, Education, and Licensure . None beyond the basic requirements.
(b) Assignment. Pharmacists at this grade level serve in a developmental capacity.
GS-12 Clinical Pharmacis! t (Full Performance Level)
(a) Experience or Education. In add! ition to the basic requirements, candidates must meet one of the following:
1. 1 year of experience equivalent to the next lower grade level, or
2. Completion of an ACPE-accredited Pharm.D. program.
(b) Assignment. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making
recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; provid! ing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.
Knowledge of professional pharmacy practice.
Ability to communicate orally and in writing to both patients and health care staff.
Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
