The Clinical Pharmacy Specialist (CPS) has multiple roles in the Hepatitis C clinic including educator, clinician, and mentor. The ultimate goals of the Hepatitis C Clinical Pharmacist services are to achieve the best clinical response, enhance patient safety, minimize treatment toxicity and implement strategies to assure the most cost efficient treatment for individual patients. This is accomplished through intensive patient education prior to treatment initiation, ongoing assessment, counseling during treatment and rigorous management of therapeutic related complications to increase the chances of a successful treatment course. The CPS will provide these services at Baltimore and will continue to mentor and educate providers and pharmacists in remote locations who collaborate with the Hepatitis C Cl! inic to manage Hepatitis C treatment for patients in their remote areas! .
In addition to direct patient care responsibilities, the Clinical Pharmacy Specialist will conduct local medication use evaluations, contribute to nationally sponsored medication use evaluations, assist with medication cost avoidance initiatives, coordinate formulary conversions, promptly report drug events to the VA Adverse Drug Event Reporting (VADERS) Program, conduct or collaborate on Hepatitis C treatment research projects, and provide student/resident teaching related to rapidly evolving Hepatitis C treatment.
In addition to the basic requirements, completion of an ASHP Accredited PGY-2 Ambulatory Care Practice Residency, and at least one year of experience in Hepatitis C management is highly desired.
NOTE: The Professional Standards Board (a peer-review group) will review the selected candidateÂ's education and experience and recommend the step at which the candidate will be placed in that specific grade. The salary is based on the! grade and step approved for the selected candidate.
QUALIFICATIONS REQUIRED:BASIC REQUIREMENTS:
a. Citizenship. Citizen of the United States.
b . License. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
c . Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
d. Physical Requirements. Scope of the Examination: The examination will be concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and will include requisite laboratory and other screening as required by Federal regulatory agencies
e. English Language Proficiency. Must be proficient in spoken and ! written English
GS-13 - Clinical Pharmacy Specialist
! Assignments - The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
Ex! perience or Education. In addition to meeting the qualifications at the GS-12 (one year of specialized experience at the next lower grade level (GS-11) or Completion of an ACPE-accredited Pharm.D. program) you must have 1 year of experience equivalent to the next lower grade level (GS-12).
Examples of assignments/duties at the GS-12 include but are not limited to
Handle routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related ! problems; contacting providers as appropriate; documenting recommendati! ons and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Have knowledge of professional pharmacy service, the ability to communicate orally and in writing to both patients and health care staff, the knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security, and the skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
In addition to meeting! all of the above requirements a Clinical Pharmacy Specialist must be able to demonstrate the followingÂ
a. The ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Have an expert understanding of regulatory and quality standards for their program area.
c. Have the ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Have expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. The advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
