The Nurse Practitioner has responsibility for continuous, coordinated and comprehensive care of Cigna-HealthSpring members at designated institutional sites (Long Term Care centers).
As coor! dinator of the care management team, the nurse practitioner as! sumes responsibility for collaborative practice with the member's primary care physician, as that physician defines it. In addition, the nurse practitioner serves as medical liaison to the interdisciplinary staff and allied health providers, provides case management services, integrates components for care conferences and case presentations, and communicates information and reports to assure seamless delivery of services to members of the interdisciplinary team that include the primary care physician, nurses, therapists, social workers, dieticians, pharmacists, and members and their responsible parties.
As a key member of the clinical team in the facility, the nurse practitioner provides preventive services, establishes medical diagnoses for both chronic and common acute problems, establishes a plan of care with short-term and long-term goals, orders and interprets diagnostic tests, prescribes medications, refers to specialists and other providers as appropriate, p! rovides basic emergency care if necessary, and acts as a resource to members, families, and staff. The nurse practitioner also assists with transitions between health care facilities and provides pertinent health care information to receiving facilities.
Demonstrates both autonomy and proficiency in performance of comprehensive historical, social, physical, and functional assessments.
Pursues therapeutic interventions and diagnostic reasoning that reflect current, evidence-based nursing and medical knowledge and practice.
Collaborates with member's physician to create a proactive plan of care (Member Care Plan) focused on early prevention, detection and timely intervention.
Develops an individualized comprehensive plan of care that aims to reduce the risk of adverse health outcomes and unplanned discharges from the nursing home.
Facilitates a medically appropriate and effective plan of care to avoid unnecessary duplication of tes! ting, prolonged hospitalization, iatrogenic illness, and fragmentation ! of care.
Demonstrates awareness of internal and external customers by following established Bravo Health guidelines for pre-certification, authorization, nursing facility stays, specialty services, and use of authorized service vendors.
Strategizes with physician and management team to enhance cost effectiveness of services while addressing continuing needs of member, maximizing member's health potential and supporting corporate mission.
Coordinates with social services for appropriate community and supportive resources across all settings of care.
Qualifications
- Active and unrestricted NP license in state of Maryland (licensure in Adult, Family or Gerontology required)
- Must possess or be eligible for DEA licensure and CDS; BCLS certification required
- Clinical competence and understanding of evidenced-based practice.
- Proficient in basic computer skills, such as sending e-mails, saving electroni! c files, retrieving saved documents, and searching the internet.
- Clear commitment to the care of the elderly and the attendant needs of their families
- Strong interpersonal and communication skills; Superior organizational skills
- Ability to multi-task; Enthusiasm, flexibility and a sense of humor
