ROLE SUMMARY
Clinic Setting: Diagnoses and treats the frail elderly and chronically ill patients experiencing episodes of acute exacerbations and monitors chronic disease progression in an urgent care setting with cardiac monitoring and IV therapy. Supplements Primary Care Provider Disease Management activities to stabilize patients and gain control of disease processes as a means of avoiding unnecessary hospitalizations. Documents all diagnoses wit! h the highest degree of specificity, ensuring that all aspects of disease progression and manifestation are documented annually. Participates in multi-disciplinary meetings and recommends treatment alternatives designed to keep patients healthy or resolve on-going clinical issues. Communicates all services provided with the patient's Primary Care Physician. Discharge plan for continuing care management is developed and communicated to ensure continuity of care. May assist with the management of skilled nursing facility and hospice patients that seek care in the medical clinics. Works closely with onsite Behavioral health, social work, and pharmacist staff to provide comprehensive care.
Minimum Requirements:
Active and unrestricted medical license in state of Maryland required
Active Board Certification in Internal Medicine required
DEA license required
ACLS certification required
For Inpatient Hospital care, admitting ! privileges at appropriate hospital.
At least five years experi! ence in a health care related setting. Prior geriatric experience required. Knowledge of chronic disease management programs and services and insurance/managed care processes and populations. Adept at marketing and public relations. Ability to travel within market locations.
Preferred Requirements: Background in inpatient care, geriatric, and/or palliative care. Experience with electronic medical record system, physician’s practice operations, health plan operations, or medical management setting.
Schedule: Per Diem; no guarantee of hours (anticipated hours are 10-20 per month)
Advanced Care Center Hours of Operation:
Monday â€" Friday 9 AM â€" 6 PM; Saturday 9 AM â€" 4 PM
