Ensures safe, appropriate, cost effective medication therapies for patients according to established po! licies, procedures and protocols.
Doctorate OF Pharmacy (Required). ACLS Certification (Preferred). Anticoagulation (Preferred). Pharmacotherapy (Preferred).
Other Education: ACPE Accredited School of Pharmacy (Required)
2 - 4 years Clinical Practice (Required). 1 year ASHP Approved Residency (Preferred). 4 - 6 years Clinical Practice (Preferred)
