What You Will Do:
General Summary
Under general supervision, provides and coordinates the care provided to transplant patients in the post transplant phase of care. Serves as a fully effective and participative member of the multidisciplinary patient care services team. Demonstrates and models the full range of professional practice in the delivery of patient care and in participation of the development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness. Is accountable to patients and families for the processes ! and outcomes of care during the post-transplant phase of care.! Is accountable to peers for the advancement of group practice within the Department of Transplantation.
Principal Responsibilities and Tasks
The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.
1. Provides comprehensive patient care to and coordinates multidisciplinary care for solid organ post transplant patients.
A. Working directly with the multidisciplinary care team, coordinates inpatient and post transplant care of transplant recipients
B. Triages all post-transplant outpatient phone calls, guiding patient care through coordination of multidisciplinary aspects.
C. Coordinates scheduling of all post hospitalization medical procedures, diagnostic testing, clinic visits, and medicine changes deemed necessary by physician! and/or protocol.
D. Provides direct support to physicians and patients after normal business to provide emergency assistance as needed.
E. Coordinates patient transportation and admission to hospital when deemed necessary by physician and or protocol.
F. Notifies transplant team members of post-transplant admissions, including admitting personnel, transplant resident, referring physician, and patient placement center.
2. Provides ongoing post transplant care maintenance.
A Under physician supervision, assists patients with medication management, prescription refills, and authorizations as required.
B. Reviews laboratory studies and reports abnormal results to physician.
C. Interacts closely with social worker, nutritionist and pharmacist to communicate patient needs and to assure that all patient needs are met.
3. Coordinates data collection for post-transplant recipients
A. Completes all Unit! ed Network for Organ Sharing (UNOS) forms required post-transplant

B. Obtains and organizes patient information in transplant database.
4. Provides appropriate nursing units with transplant educational materials and continual updates as needed or required.
5. Provides ongoing patient/family education and support
A. Provides patient education and pertinent transplant information to post transplant recipients.
B. Assists in the development of patient and family educational materials. Ensures that all material is updated as new information becomes available.
6. Participates in patient-related consultations with physician staff as requested, including attendance at multidisciplinary meetings/rounds.
7. Establishes and maintains frequent contact with referring physicians and institutions to ensure the timely transmission of pertinent medical information.
A. Communicates with referring physician immediately post-transplant.
B. Communicates during the post-transplant out! patient phase as necessary
10. Assists in the development of all transplant related clinical protocols.
11. Assists the Medical Director in a variety of areas to facilitate the Transplant Program.
12. Assists with promotion of the transplant program through interaction with referring physicians and education of dialysis unit nurses and staff.
What You Need to Be Successful:
1. Licensure as a Registered Nurse in the State of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required. BSN preferred.
2. Dialysis, Critical Care or Transplant experience is preferred.
3. Must obtain NATCO CCTC certification within 2 years of hire, or within a time frame determined by management. Must maintain NATCO certification once received.
