The successful candid! ate will be able to:

  • Coordinate and facilitate new referrals/admissions through collaboration with nursing, customer service and distribution.
  • Review, receive/verify all new patient orders and documents the same on the respective order form.
  • Assist the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy.
  • Conduct initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.
  • Assist the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy.
  • Monitor the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data other pertinent patient information.
  • Communicate the results of therape! utic drug monitoring activities to the physician, nursing and other appropriate persons.
  • Program infusion control devices as indicated.
  • Provide consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
  • Maintain thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians and other healthcare professionals.
  • Supervise, coordinate and/or participate in the preparation of sterile admixtures, dispense all pharmaceutical and related supplies and equipment in accordance with federal, state and local laws and standards of practice which govern the profession of pharmacy.
  • Process and check all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Maintain pharmacy records in accordance with applicable laws and regulations.
  • Participate in on-call rotatio! n.

What Else You'll Bring to the Table
In addition to the competencies detailed above, you'll need:

  • A Bachelor of Science in Pharmacy (PharmD is highly desirable) with a minimum of 3-5 years pharmacy operations and clinical practice experience in home infusion therapy.
  • Currently licensed as a registered pharmacist in state of employment and other states as required by law.
  • Possess a strong knowledge base in intravenous admixture and clinical aspects of home infusion therapy.
  • Possess the necessary supervisory/management skills and/or experience to supervise the pharmacy operations of home infusion therapy.
  • Demonstrated ability to communicate effectively both verbally and in writing.
  • Demonstrated ability to work well with others.
  • Possess good organizational and time-management skills. Possess the ability and initiative to plan for and complete daily activities with minimal direction.
