Clinical Specialist â€" Adult Internal Medicine: The Adult Internal Medicine Clinical Pharmacy Specialist will be responsible for the provision and oversight of ph! armacotherapy for adult internal medicine patients. The clinical pharmacy specialist will support physicians, nurses, pharmacists and other care providers in the management of patients requiring acute care within the hospital. Further, this practitioner will provide continuity of care and post discharge follow up services for these patients to support our evolving care transitions model. They will provide drug information and education to patients, students, residents, pharmacists, and other health care practitioners.
They will serve as a preceptor for pharmacy residents of ASHP accredited PGY-1 general practice and PGY-2 specialty practice programs. Faculty appointments are available through affiliations with the College of Pharmacy at the University of Maryland and the Johns Hopkins University School of Medicine. Additionally, the successful candidate will support drug research conducted by the institution and seek to assume a leadership role in the profession of! pharmacy. Preferred qualifications for this position include ! the completion of the PharmD degree and a specialized residency or fellowship in Pharmacotherapy or Adult Internal Medicine.
