Responsibilities:
Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.
Recommends formulary alternatives when appropriate.
Compounds and dispenses medications for accurate delivery to patients.
Provi! des drug information and education to medical staff and patien! ts.
Communicates with physicians and other clinicians as necessary to facilitate care.
Education & Experience:
Achieves and maintains required skill recognition status within six (6) â€" twelve (12) months of initial hiring required.
Requires a solid therapeutic knowledge with respect to pharmacology, drug information, pharmacokinetics, drug interactions, and regulatory requirements and a specialized knowledge of essential policies and procedures pertinent to the medication utilized in the treatment of oncology patients and an in-depth understanding of the technical specifications for the safe and sterile preparation of chemotherapy medications at date of hire.
One (1) year hospital experience in Oncology Pharmacy. ASHP Oncology Practices and IV Preparations Standards at date of hire preferred.
Graduate of an accredited school of pharmacy with a five-year college program in pharmacy or a six-year PharmD Program at date of hire required.
Licenses & Certifications:
Board Certified Pharmacotherapy Specialist - Oncology Pharmacy at date of hire preferred.
Licensure as registered pharmacist by the Maryland State Board of Pharmacy at date of hire required.
